Medical Authorization

All clubs should have on file a medical authorization slip signed by the athlete and a parent or guardian granting permission for emergency medical treatment. The form should list emergency contact information. A duplicate copy should be with the coach at every practice and competition.

MJVBA Championships will be using Advanced Event Systems for its registration process. AES will be the website that is the choice of many regional & national tournaments around the nation for registering teams.

 

When done you will return to www.mjvba.com/statetourney  to pay for entries.

 

 

Important Notes before beginning process

 

  • You will be asked to enter a CLUB CODE this will be your 5 alpha MJVBA ID. The one you have been using all year.  Not your National AAU MIVB …….. code
  • If you do not remember you MJVBA Club ID. It can be found on www.mjvba.com/eligibleclubs.htm
  • When asked to pick a region from the drop down menu select MJVBA (MJ)
  • When asked for USAV # select pending
  • ROSTERS are required for 15-18 to enter this event. Rosters be used to create a recruitment packet for college coaches.  
  • Rosters may be updated on line until 5/22 – this is for the recruitment packet only.
  • When you check-in at the tournament the MJVBA roster that you turn in will be the official roster for the event.

 

  1. Go to Advanced Event Systems to create your club, click Create New Account.
    1. **In an effort to increase data integrity we ask that you create your club again for past users (prior to 08-09 season).   
  2. Next, fill out Create Club Director Account form.  Keep your username and password in an accessible spot for future use.
    1. You will be asked to enter a CLUB CODE this will be your 5 alpha MJVBA ID.
  3. Click on Add New Teams, enter info, and Submit information.

    2. Add Players and Staff into the system by clicking the appropriate button on left– enter/add or import data.
  4. To Register: Click "Enter Events." Hit the "Tournament Name – MJVBA Association Championships
  5. Select the division from the pull down list that you wish to enter your team and click on "Register Teams."
    1. BUILD ROSTER – add staff/players to your event roster, click UPDATE ROSTER, click Continue.  If Rosters are not required, click Continue
    2. To update event roster, click My Events, My Teams, click Roster.
  6. MJVBA will send out broadcast emails to inform you of other information or changes as they come available.

To pay on-line - click here on-line payment. The payment will go through PayPal but you do not need a PayPal account to pay on-line.
